What can you expect?
In a medical home, you and your team will work together. As an active member of the team, you will have
a chance to explain the things that are really important to you.
- Your team can answer questions and help you better understand your health care needs.
- If you need to get help from other doctors, your team can support you every step of the way.
- When you have concerns about your health, your medical home team will work with you to determine the best way to deal with them.
Together, you and your team can work on a plan that:
- Is personalized or created just for you
- Is coordinated with other health care providers
- Connects you with your health care team
Working with your team may improved the quality of your health care and shorten the time it takes to get that care.
Hours of Operation
Appointments

Because your time is as valuable
as ours is, we see patients on an appointment basis. We feel this means less waiting
for you. For this to work, however, we need your cooperation. We strongly encourage
you to call before coming in and to give as much information as possible. This allows
us to set aside enough time for you and to arrange better staffing for your specific
problem(s).
We do see “work-ins” but discourage “walk-ins”. Essentially this means we will be
glad to see you but please call before coming in if at all possible. Again, this
allows us to be better prepared to serve you.
Initial appointments take more time to gather medical and non-medical information.
You may print patient registration forms ahead of time and bring with you to your
visit. This will help to save time in preparing information for your chart. These
are located on our website under the Patient Forms button, Patient information and
Patient history.
We strongly prefer to have old records, either sent to us ahead of time or brought
in by you at the time of your first visit. Doctor offices and hospitals require
written permission to send copies of your records to us. You can obtain a “Record
Request Form” located under Patient Forms on this website or from our front desk.
Canceling Appointments: If you are unable to keep an appointment, please call us.
This simple courtesy is much appreciated and permits time for others trying to be
seen. We will work the best we can to accommodate your schedule.
Phone Calls

We
realize there may be times that you have a question or concern about an illness
or medication. Our reception staff will be happy to take a message and forward to
the physician or triage staff. Unless your need is an emergency, it may take several
hours before our staff contacts you.
We need you to furnish your phone number, date of birth and pharmacy of choice along
with your message. This helps to expedite your call.
Triage phone calls after 4:30PM will be handled the following day, unless it is
an emergency. Please call for all refills several days before running out of medication.
No prescriptions for pain medicine or nerve or sleep medicine will be filled on
night or weekends.
